In the video we performed a laparoscopic ipom plus repair for a right 3 cm defect Spigelian Hernia. The patient is a 63 y.o. female without any important comorbidity. She presented with a 5 cm palpable not reducible tender mass in the right iliac fossa. We made a periombelical open laparoscopic access, 5 mm trocar in FISx and 10 mm trocar in left hypochondrium. At the laparoscopic exploration there was a spigelian right hernia with omental content and a fascial defect of about 3 cm x 3 cm. After the reduction of the hernia content, we closed the defect with 2/0 Vlock running suture. Ventralight 15 cm x 20 cm non absorbable mesh was placed using ReliaTack fixation device.
